The Hillcrest Homeless Outreach Program is a federally funded grant program that has served over 100 people annually in the Dubuque area. Funding is received through a federal Substance Abuse and Mental Health Services Administration grant, Projects for Assistance in Transition from Homelessness (PATH). Eligibility requirements are (1) (a) individuals suffering from serious mental illness; or (b) are suffering from serious mental illness and have a substance use disorder; and (2) are homeless or at imminent risk of becoming homeless. When can outreach help: Living in Transitional housing Living in a short term or long term shelter Living with friends or family members on an emergency basis Being released form jail/prison, hospital or mental health facility with no place to go Living on the street Staying in an abandoned building Sleeping in a car Living in any place not meant for habitation Ways others in Community can help: Donate
