The Hillcrest Homeless Outreach Program is a federally funded grant program that has served over 100 people annually in the Dubuque area. Funding is received through a federal Substance Abuse and Mental Health Services Administration grant, Projects for Assistance in Transition from Homelessness (PATH). Eligibility requirements are (1) (a) individuals suffering from serious mental illness; or (b) are suffering from serious mental illness and have a substance use disorder; and (2) are homeless or at imminent risk of becoming homeless.

Services available:

1. Outreach
2. Screening and diagnostic treatment
3. Habilitation and rehabilitation
4. Community mental health
5. Alcohol or drug treatment
6. Case management services
7. Assistance in obtaining income support services (housing, food stamps, SSI, etc.)
8. Housing services including searching for apartment security deposit, household items and furniture
9. Help with prescription medication
10. Referrals to other agencies/programs, as needed.

When can outreach help:

•  Living in Transitional housing

•  Living in a short term or long term shelter

•  Living with friends or family members on an emergency basis

•  Being released form jail/prison, hospital or mental health facility with no place to go

•  Living on the street

•  Staying in an abandoned building

•  Sleeping in a car

•  Living in any place not meant for habitation
