Title:Remedial Service Provider - Maquoketa & Washington, IA
Status:Contract
Purpose:To provide remedial, crisis and child welfare services to children, youth and their families.
Qualifications:Bachelor’s degree in psychology, social work or related field, plus a minimum of two years experience working in community based services with children, youth and families.
Reportability:Supervised by Program Manager/Lead Therapist.
Scope:Mentoring, teaching and coaching skills in order to provide information, education, support, direction and oversight of children and youth with a mental health diagnosis and their families.
Skills:The Remedial Services Provider must have thorough knowledge and understanding of developmental characteristics of children and youth; competencies in the delivery of evidence based individual, group and family services; understanding of family systems and strategies to positively impact change within those systems and the appropriate provision and documentation of Medicaid eligible remedial and crisis services.
Relationships:Ability to self-monitor and provide acceptable remedial service plans and documentation, monitor adherence to the child’s LPHA order, and monitor outcome data and make adjustments, as necessary.
Principle Responsibilities:
To write the child’s remedial service plan according to the diagnosis, evidence based practices in the treatment of children with a specific mental health diagnosis or symptoms and collaboration with the child’s family and other community supports. The plan must be written in conjunction with an LPHA order within 48 hours of the receipt of the order. The plan must be accepted by IME prior to implementation.

To provide high quality remedial services to individuals, groups, and families eligible for remedial services. An average of 75 hours of individual remedial services must be completed each month. Any combination of individual and group remedial services that meet this standard is acceptable.

To complete all documentation in an acceptable manner, according to Medicaid, state and Hillcrest guidelines.

To complete Remedial Service plans and documentation within the UNISON system.

To represent Hillcrest at DHS, JCS and other related community meetings in order to keep abreast of any proposed changes to the remedial and/or the child welfare service system as directed by the Remedial Services Supervisor.

To coordinate the delivery of remedial or child welfare services with any/all other services provided by Hillcrest or any external provider.

To attend a minimum of 24 hours of acceptable training within the first year of employment and a minimum of 15 hours annually after the first year.

Physical Requirements:
The Remedial Services Provider must be able to sit, stand, stoop and crawl. Ability to lift up to 25 lbs.
