Title:Director of Community Health Services
Status:Exempt
Purpose:To direct and supervise the Community Health Services Programs including Maternal Health, WIC and the Professional Health Clinic.
Qualifications:Master's degree in health services field with experience in health care delivery, or the minimum of a Bachelor's degree in a health field with at least two years progressive supervision, and successful grant writing experience and management required.
Reportability:The Director of Community Health Services reports to and is directly accountable to the Vice President of Human Development.
Scope:This position has impact upon the staff at the Professional Health Clinic and WIC, as well as clients of those programs.
Skills:The Director of Community Health Services must possess a knowledge of the clinical practice of family and individual health services. The person must also possess the knowledge of concepts related to the administration of public health services, particularly to women and children. The person must also have knowledge and skills in successful grant preparation, budgeting and evaluation. Management skills in the areas of personnel fiscal management, data aggregation and evaluation are essential. A minimum of two years previous experience in supervision is essential. The person must demonstrate clear, tactful and professional communication skills to staff, clients and other health and human service professionals. The person should have the initiative to seek out new program ideas while regularly reviewing current methods. The Director of Community Health Services must be able to objectively determine what is in the best interest of clients, and be able to mediate differences of professional opinion among staff and community providers. All Hillcrest policies and procedures must be followed.
Relationships:The Director of Community Health Services is a member of the Hillcrest Leadership group. He/she participates in leadership groups for the Family Planning Council of Iowa and the Iowa Department of Public Health (WIC and Maternal Health). He/she will also participate in other state and regional committees as approved by the Vice President of Human Development. The person must develop and maintain open lines of communication with all administrative, support, supervisory and direct care staff internally, and with referral agencies and those professionals providing service or consultation to the Community Health Services Programs.
Principle Responsibilities:
1. To direct, supervise and promote the operation of community health programs.
a. Attend and facilitate meetings of program staff as deemed appropriate.
b. Provide specific program direction to staff.
c. To be liaison with agencies providing funding to and/or purchasing services from Hillcrest Community Health Service Programs.
d. To prepare for and present information for compliance site reviews of Family Planning, WIC and Maternal Health Programs.
e. To meet regularly on individual supervision basis with lead staff and with entire units as necessary.
f. To present the programs to the public through public speaking and media interviews.

2. To assess the effectiveness of present programs and direct new initiatives.
a. To regularly review protocols and planning processes for service delivery to ensure that programs meet client needs, needs of the community and standard practice criteria.
b. To develop annual goals and objectives for each program within Community Health Services.
c. To prepare written recommendation for program and staff changes.
d. To monitor written recommendation for program and staff changes.
e. To ensure compliance with Title X, HIPAA and other applicable state and federal regulations.

3. To attend to Community Health Service fiscal issues.
a. To recommend line item amounts for annual budgets for each program, inclusive of salary adjustments.
b. To monitor and adjust budget items throughout the year.
c. To approve expenditures.
d. To monitor inventory of supplies.
e. To assist fiscal officer in budget modification for Iowa Department of Public Health and Family Planning Council of Iowa as needed.
f. To write grants and project proposals for existing programs and for new projects that are undertaken and monitor their compliance when approved.
g. To receive final approval for these grants from the Vice President of Human Development and/or Vice President of Mission Advancement.

4. To provide supervision and direction to staff immediately responsible to the Director of Community Health Services.
a. To interview applicants and recommend candidates for hire.
b. To provide timely annual performance evaluations and receive the approval of the Vice President of Human Development before delivery.
c. To provide for ongoing professional staff development.
d. To recommend conferences and workshop attendance.
e. To administer disciplinary action and provide recognition to staff as appropriate.
f. To recommend discharge and suspension to the Vice President of Human Development when necessary.

5. To oversee the staff hiring and development of all other Community Health Service staff.
a. To approve the hiring of other staff positions.
b. To approve conferences and workshops for all Community Health Service staff.
c. To recognize staff for their accomplishments.

6. To contract for those services in WIC, Maternal Health and Family Planning that are best provided on a "fee for service" basis (including Dental Hygienist, Medical Director, R.N. for WIC certifications).
a. To negotiate a contract with the consultants.
b. To seek final approval of the contract with the Assistant Executive Director/COO and the Assistant Director of Finance.
c. To annually review the performance of the consultants in conjunction with program staff and make recommendations on renewal of the contracts.
d. To troubleshoot issues between the consultants and program staff when these issues cannot be handled between the parties.

7. To coordinate the placements of students in Community Health Service Programs.
a. To approve the placement of social work interns and students in health related fields and assign primary responsibility for supervision to the appropriate staff.
b. To ensure that interns receive the appropriate orientation, direction, supervision and evaluation.

8. To direct the adherence to all relevant licensure regulations, contract language, and accreditation standards.
a. To keep abreast of developments within licensure and accreditation standards and inform appropriate staff.
b. To be liaison with licensure personnel on standard interpretation.
c. To develop a method for annual self-assessment of licensure and accreditation compliance.
d. To delegate responsibilities for standards compliance and follow-up for identified deficiencies.

9. To assume other duties as assigned by the Assistant Executive Director/COO.
a. To develop discrete job tasks for the assigned responsibilities.
b. To report back to the Assistant Executive Director/COO on project status.
c. To report regularly to the Assistant Executive Director/COO on routine and non-routine responsibilities and issues.
d. To submit monthly written reports to the Assistant Executive Director/COO if requested.
e. To be a contributing member of the Quality Council of Hillcrest Family Services.

B. Project Director for Maternal Health.

The Project Director is responsible to the director of the local agency and receives technical assistance from staff of Bureau of Family Health. The Project Director has administrative and supervisory responsibilities for the following areas of the program.

10. Grant Development
a. Coordination among local MCH/FP programs.
b. Interpreting and complying with all state and federal guidelines.
c. Developing the agency budget in compliance with state and federal guidelines.
d. Developing agency quality assurance plans including providing leadership for the agency chart audit team and related monitoring activities.
e. Interpreting data system reports.
f. Participating in community needs assessment in the agency service area.

11. Contract Management
a. Attending meetings and workshops.
b. Monitoring compliance with state and federal guidelines.
c. Overseeing accurate completion of all agency reports and records.
d. Overseeing contractual relationships with health providers and providing training to these groups.
e. Ensuring timely submission of Maternal Health data as outlined in the contract.
f. Providing written notice of contract changes.
g. Ensuring progress on program plan on file with the Department.

12. Program Development and Coordination. The Project Directorˇ¦s responsibilities for program development and coordination include: Supervising Program Coordinators to:
a. Ensure integration of services.
b. Ensure coordination with and referrals to family planning agencies.
c. Promote childhood immunizations for program participants.
d. Ensure referral of MCH participants to other sources of health care as needed.
e. Provide documentation and follow-up of all referrals.
f. Assisting the County Board(s) of Health in the performance of the core public health functions of assessment, assurance, and policy development for the maternal and child health population.

13. Outreach
a. Serving as a health professional representing the MCH program to the public.
b. Providing program information to interested professionals, outside agencies, organizations and individuals.
c. Directing MCH outreach activities and assuring an effective referral system.

14. Agency/Program Coordination Systems. The Project Directorˇ¦s position may require communication or interaction with, but not limited to:
a. Iowa Department of Health.
b. Department of Human Services.
c. Public Health Nursing.
d. Area physicians, HMOs and hospital staff.
e. Empowerment.
f. Area Education Agency.
g. Early ACCESS.
h. Family Planning agencies.
i. Lead Poisoning Prevention programs.
j. Child Health Speciality Clinics.
k. Head Start.
l. Child Care Resource and Referral agencies.
m. Other community groups.
n. WIC program staff.

15. Supervisory

The Project Directorˇ¦s supervisory responsibilities include:
a. Oversight to organization and management of all clinic sites, including determination of clinic sites, hours of service, and scheduling issues.
b. Supervising both professional and non-professional agency staff.
c. Recruiting, training, and monitoring all personnel, including outreach workers and project volunteers as they are involved in the MCH program.

C. WIC Coordination.

The program coordinator has administrative and supervisory responsibilities for the following areas of WIC program management. 1

6. Grant development.
a. Coordinating the local WIC program.
b. Interpreting and complying with state and federal guidelines.
c. Developing the agency budget in compliance with state and federal guidelines.
d. Functioning as a member of the agency chart audit team.
e. Interpreting data system reports.
f. Participating in community needs assessment in the agency service area.

17. Contract Management.
a. Attending meetings and workshops relevant to program operations.
b. Monitoring compliance with state and federal guidelines.
c. Managing the WIC program efficiently while fulfilling all grant specifications.
d. Overseeing accurate completion of all agency reports and records.
e. Overseeing contractual relationships with health providers and vendors and providing training to these groups.
f. Conducting vendor monitoring as directed by state personnel.

18. Adjunct to health care services.
a. Ensuring integration of services with Maternal Health and Child Health.
b. Ensuring coordination with and referrals to family planning agencies.
c. Promoting childhood immunizations for program participants.
d. Ensuring referral of WIC participants to other sources of health care as needed.
e. Providing documentation and follow-up of all referrals.
f. Assisting the County Board(s) of Health in the performance of the core public health functions of assessment, assurance, and policy development for the maternal and child population.

19. Outreach
a. Serving as a health professional representing the WIC program to the public.
b. Providing program information to interested professionals, outside agencies, organizations and individuals.
c. Directing and evaluating the effectiveness of WIC outreach activities.

20. Supervisory
a. Organizing and managing all clinic sites, including determination of clinic sites, hours of service, and scheduling issues.
b. Arranging for transfer of needed equipment, forms, records, and checks to clinic sites on appropriate days using appropriate measures to safeguard personal health information.
c. Supervising both professional and non-professional agency staff.
d. Recruiting, training, and monitoring all personnel, including outreach workers and project volunteers as they are involved in the WIC program.
e. Working with agency nutritionists and nurses and state personnel to provide in-service training to project staff about policy changes and changes in the science of health and nutrition that influence health, nutrition, and food education activities.

21. Oversight

Physical Requirements:
This position requires bending and stooping and reaching above shoulder level occasionally. Must be able to lift up to 50 pounds. 10% of time spent standing, 20% of time spent walking and 70% of time spent sitting. Must have average hearing and visual acuity. Is required to use a phone and computer and have average manual dexterity. Must be able to operate vehicles. May be exposed to common household cleaning chemicals and noise through the clinic.
