| Purpose: | To provide couple, individual and family therapy to clients seeking help in resolving intrapersonal and/or interpersonal conflicts and/or situational stresses in their lives. |
| Qualifications: | Master's degree in social work, counseling or related field. Must be eligible for licensing in Iowa: LISW, LMHC, or LMSW. |
| Reportability: | Therapist reports to the Lead Therapist. |
| Scope: | To provide direct care services to a community health center. |
| Skills: | The therapist must possess a knowledge of the theories and techniques of therapy. The person must be able to apply these in the clinical setting in the provision of ongoing services to families and individuals. The person must be knowledgeable of growth and development issues specific to children, adolescents, young adults, middle adults and older adult age-groups. The person must be able to communicate clearly in speech and writing to other professionals and to clients.
Must be capable of learning basic computer skills for the implementation of patient/client information electronic record keeping. The therapist will be aware of current trends and issues in the human services and make appropriate use of this information as a therapist. Therapists are expected to follow Hillcrest policies and procedures. Must be able to visually observe patient/client needs. Possesses the ability to work productively and effectively. |
| Relationships: | The therapist will develop and maintain open lines of communication with all administrative, support, supervisory and direct care staff internally and with referral agencies and those professionals providing services or consultation to Hillcrest. |
| Principle Responsibilities: | |
| 1. Delivers therapy services to clients (individual and/or group) according to state and agency guidelines. 2. Responsible for an average of 100 billable hours per month. (Part time position will be responsible for a pro-rated number of billable hours, adjusted for the number of hours scheduled to work.) 3. Completes psychosocial evaluations on assigned patients/clients in a timely fashion. Displays excellent interviewing, diagnostic and report writing skills. 4. Interviews patients/clients, family members and others to identify the precipitating event and degree of disruption that the person is suffering. 5. Assesses the impact of psychosocial stressors in order to minimize the negative, destructive outcome of a crisis experience. 6. Possesses excellent theoretical knowledge of psychopathology and treatment techniques of each type of therapy. 7. Is knowledgeable of facility and community resources. Collaborates with medical staff and community-based agencies regarding evaluation, recommendation and referral disposition. (Includes DHS, JCS, law enforcement and other Hillcrest departments). 8. Performs all aspects of patient/client care in an environment that optimizes patient/client safety and reduces the likelihood of medical/health care errors. 9. Correctly utilizes required outpatient forms and maintains accurate and orderly case records, including initial assessment, mental status exam, diagnosis, initial treatment plan and reviews, progress reports, and discharge summaries (if applicable). 10. Demonstrates appropriate levels of software utilization to support documentation requirements, agency policy and procedures, and quality service delivery. 11. Documents individual casenotes within 24 hours of each session, by either dictation, Unison or handwritten notes as required for the medical record and third party payer sources. 12. 12. Ability to complete initial assessments which contain a complete social history and a diagnosis supported with symptoms.br> 13. Completes treatment plans which summarize assessments and link patient/clients to the appropriate level of care no later than after the second therapy session which contain goals that are specific, measurable, personalized, and achievable. br> 14. Acts as an advocate for the patient/client in the community. 15. Maintains a good working relationship within the department and with other departments. 16. Communicates appropriately and clearly to the Program Director, co-workers and medical staff. 17. Provides a safe environment for patients/clients and staff, including being aware of and documenting unusual incidents. 18. Treats patients/clients and their families with respect and dignity. 19. Is emotional and mentally stable to work with the program’s clientele, model self-care skills and assist patient/client in meeting his/her goals. 20. Demonstrates the ability to be flexible, organized and function under stressful situations. 21. Demonstrates knowledge of and appropriate use of informed consent, confidentiality and client’s rights. 22. Responds to emergency calls from current or potential clients, demonstrating the ability to provide crisis intervention or support services. 23. Demonstrates the ability to be timely with regard to returned client phone calls (within 24 hours or receipt) and submitting accurate billing forms to financial office. 24. Obtains legal authorization to release or obtain information pertinent to each individual case. 25. Serves as a mental health resource to the agency and the community. 26. Participates in staff meetings and case conferences. 27. Represents Hillcrest's interest in community groups at the discretion of the MHC Clinical Director and shares information as requested. 28. Increases skill level through continuing education and work with diverse clients. 29. Participates as a member of a multi-disciplinary team. 30. Refers clients for medication services, psychological testing, therapy groups, and other Hillcrest or community services as clinically indicated. 31. Works cooperatively with support staff to resolve scheduling or billing issues. 32. Reviews client progress with other treatment providers as needed. 33. Complies with facility’s policies and procedures and safety management. 34. Keeps current with advances in the therapy field through continuing education, seminars/conferences, professional societies and journals, etc. 35. Completes other duties as necessary and assigned by Program Director and/or Chief Executive Officer. | |
| Physical Requirements: | |
| Must be able to lift 25 pounds. 15% of time spent standing, 10% of time spent walking and 75% of time spent sitting. Must have average hearing and visual acuity. Is required to use a phone and have manual dexterity.
