Our Commitment to Privacy
Your privacy is important to us. To better protect your privacy we provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used. To make this notice easy to find, we make it available on our home page and at every point where personally identifiable information may be requested.

The Information We Collect
This notice applies to all information collected or submitted on the Hillcrest Family Services website. On some pages, you can make donations, pay outstanding invoices, make requests for information, and apply for employment with our agency. Following are the types of personal information collected at these pages:

- Credit/Debit Card Information (etc.)
- Work History
When you use the public areas of our website, you are doing so anonymously.
The Way We Use Information
We do collect aggregate use information, such as the number of hits per page, for internal and marketing use only; we do not provide your personally identifying information to the public.
We use the information you provide about yourself when conducting a transaction to complete that transaction only. We do not share this information with outside parties except to the extent necessary to complete your request.
We use return e-mail addresses to answer the e-mail we receive. Such addresses are not used for any other purpose and are not shared with outside parties.
Finally, we never use or share the personally identifiable information provided to us online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses.
Our Commitment to Data Security
To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online.
Pricing Policy
Hillcrest Family Services is a non-profit organization with many diverse programs and services. Pricing will depend on services and funding source. Please select the program on the Program and Services page and view the Contact Us section to contact the appropriate Hillcrest staff for questions regarding services for that program.
Online Donations and Online Payment of Client Statements for Services
Forms of Payment: We accept Visa, Mastercard and Discover payments. We can also accept healthcare flexible spending cards with the Mastercard logo. Checks are also accepted. There is a $25 returned check fee.
Refund Policy
Refunds on payments made on an outstanding balance due will only be made if the account has been overpaid. Please contact billing@hillcrest-fs.org for balance information.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Uses and disclosures of health information: We seek your consent to use health information about you for treatment, to obtain payment for treatment, for administrative purposes, and to evaluate the quality of care you receive. Continuity of care is part of treatment and your records may be shared with other providers to whom you are referred. Information may be shared by paper mail, electronic mail, fax or other methods. You can revoke your consent.
We may use or disclose identifiable health information about you without your authorization for several reasons. Subject to certain requirements, we may give out health information without your authorization for public health purposes, for auditing purposes, for research studies, and for emergencies. In any other situation, we will ask for your written authorization before using or disclosing any identifiable health information about you. If you choose to sign an authorization to disclose information, you can later revoke that authorization to stop any future uses and discloses.
We may change our policies at any time. Before we make a significant change in our policies, we will change our notice and post the new notice in the waiting area of each service facility. Individual rights: In most cases, you have the right to look at or get a copy of health information about you that we use to make decision about you. If you request copies, we will charge you only normal photocopy fees. You also have the right to receive a list of instances where we have disclosed health information about you for reasons other than treatment, payment or related administrative purposes. If you believe that information in your record is incorrect or if important information is missing, you have the right to request that we correct the existing information or add the missing information.
Complaints: If you are concerned that we have violated your privacy rights or you disagree with a decision we made about access to your records, you may contact the person listed below. You also may send a written complaint to the U.S. Department of Health and Human Services. The person listed below can provide you with the appropriate address upon request.
Our legal duty: We are required by law to protect the privacy of your information, provide this notice about our information practices, and follow the information practices that are described in this notice.
