The Homeless Outreach Program serves individuals who have a Diagnosed Mental Health illness OR have a Diagnosed Mental Health illness and have a substance abuse disorder; AND are homeless or are in imminent risk of becoming homeless. Services available include: outreach; assistance in obtaining income support services, housing services- including assistance in obtaining shelter or apartment searching, security or utility deposit assistance (if funds available), applying for Sect. 8 housing program; help with prescription medication costs (if funds available); assistance in obtaining basic needs; referrals for other community resources, including mental health or drug and alcohol treatment and case management.
Funding for the program is provided by a federal SAMHSA (Substance Abuse and Mental Health Services Administration) grant. This project is specifically designated as a PATH (Projects for Assistance in Transition from Homelessness) program. Any interested party or agency can make a referral.
