Hillcrest Supported Living (HSL) is a program that provides an individual with a Visiting Counselor who assists them in developing supports and skills they want or need, in order to gain satisfaction in their lives. Services are provided in their own home, apartment, or in a family home, and can be provided as often as the individual requests. Currently, the program is available in Dubuque, Johnson, Linn and Delaware counties.

Visiting Counselors provide skill-building and training in all areas of daily life, including:

Decision-making

Expected outcomes of the Supported Living Program include: the development of a support network in the community; successful residence in the community; and the prevention of unnecessary hospitalizations.

Referrals are accepted from any professional or interested party. Participants must be 18 or over, have a diagnosis of a serious mental illness, and are able to live independently with some supportive supervision. An individual’s participation in the program can be paid through County funds, Title 19 CSS, Waiver and Habilitation programs, State Payment Program, or through private pay.
