HILLCREST SUPPORTED LIVING

Hillcrest Supported Living (HSL) provides a team of Visiting Counselors who specialize in assisting adults with serious mental illness, brain injury or developmental disability living in their own homes and apartments or with their families. This assistance enables the client to live independently in the community. The HSL program is available in Dubuque, Johnson, Linn, and Delaware counties.

Visiting counselors provide skill building and training in all areas of daily life, including:

  • decision making
  • money management
  • illness and medication education
  • community living skills
  • setting and attainment of goals
  • coping skills
  • developing and maintaining a daily routine
  • learning about and how to apply for community resources
  • scheduling/attending appointments

Referrals are accepted from any professional or interested party. Participants must be 18 or over with a psychiatric disability and able to live independently with some supervision. An individual's participation in the program can be paid through a Purchase of Service contract, county funds, private pay, Title 19 Waiver Program, State payment program or other sources.
